One is new and unused, other one's cable has been slightly modified. Prices of both the items

No it doesnt require a 5V reg..more over it outputs 5V which you can use to power your FPV camera

Can't comment on the range or give exact figures. Range depends on many factors: Noise in the area, how well tuned is your antenna, condition of the day. As much I can say with the cheap boscam 200mW Tx on CP antennas(TBS triumphs) I could easily fly upto 800-900m with very less signal break. So 600mW with a good patch antenna and diversity Rx will easily give you much more. Also go through this video:
